Indications
Turbid urination from cold in a deficient lower burner: without warmth the qi transformation of the Bladder fails, so clear and turbid are no longer separated and essence-like turbidity leaks away with the urine.
Clinical Manifestations
- Cloudy white urine like rice water
- Thick, paste-like turbid urine
- Frequent urination with a cold lower abdomen
- Chronic turbid discharge with no burning
- Cloudy urine in a patient who feels the cold

Common adjustments
- Kidney yang is markedly weak — add Rou Gui 肉桂 and Bu Gu Zhi 补骨脂
- Leakage is persistent — add Sang Piao Xiao 桑螵蛸 and Fu Pen Zi 覆盆子
- Qi is depleted with a bearing-down feeling — add Huang Qi 黄芪 and Sheng Ma 升麻
- Damp-heat signs appear (burning, yellow urine) — switch strategy — the damp-heat version of this formula from 《医学心悟》 is the appropriate one
Contraindications
Not for turbid urination from damp-heat, where the urine burns, is dark or yellow, and the coating is yellow and greasy — warming herbs will feed that. Persistently cloudy urine needs a medical work-up: infection, chyluria, proteinuria and prostatic disease all present this way and none should be managed as a pattern alone.

Pregnancy
Use with caution in pregnancy — the aromatic warming herbs are not routinely used in pregnancy, and cloudy urine during pregnancy should be investigated for infection before any herbal treatment.
