Indications
Exhaustion of the lower origin with turbid phlegm rising: Kidney yin and yang are both depleted, and the phlegm that fills the vacancy blocks the orifices of speech and awareness.

- Stiff tongue with loss or slurring of speech
- Legs that will not carry weight after a stroke
- Dry mouth without a wish to drink, with cold feet and a flushed face
- Weakness and cognitive dulling in an elderly patient

- Heat signs present with a red tongue — remove Fu Zi and Rou Gui 肉桂
- Phlegm heavy with a thick greasy coating — increase Shi Chang Pu 石菖蒲 and add Ban Xia 半夏
- Internal wind stirring with tremor — add Tian Ma 天麻 and Gou Teng 钩藤
Contraindications
Not for the acute phase of a stroke driven by ascending Liver yang, internal wind or phlegm-fire — the pattern here is deficiency at the root. Sudden loss of speech or limb weakness is a medical emergency: it must be assessed and imaged before any herbal treatment is considered.

Pregnancy
Contraindicated in pregnancy — it contains prepared Fu Zi and Rou Gui, both classically avoided in pregnancy.
