Indications

Cools the intestines and stops bleedingDisperses wind and moves qiRelieves rectal bleeding without leaving stasis

Wind-heat or damp-heat toxin obstructing the intestinal tract and injuring its vessels. Heat scatters blood out of the collaterals of the large intestine, so it appears with the stool.

Contraindications

Not for rectal bleeding from deficiency-cold or from qi failing to hold blood — pale dilute blood in a tired, cold patient with loose stools needs warming and tonification instead. Because it is cooling and dispersing it is not suitable for long-term use. Rectal bleeding at any age needs a diagnosis: haemorrhoids are common, but so are polyps and colorectal cancer, and the formula treats a symptom rather than its cause.

Pregnancy

Used with caution in pregnancy and only on a practitioner's direction. Haemorrhoidal bleeding is common in pregnancy, but Huai Hua is cooling and downward-moving and Zhi Ke moves qi downward, neither of which is ideal once pregnancy is advanced. Any rectal bleeding in pregnancy should be examined rather than assumed to be haemorrhoids. There is no established interaction with anticoagulant medication, though a patient on such a drug who is bleeding should be reviewed by the prescriber.
