Indications
Blood moving recklessly from blood heatwith heat but without significant deficiency. Yang has overwhelmed yin in the blood level, so blood spills upward, and the pattern is one of heat and dryness rather than of collapse.
Clinical Manifestations
- Vomiting or spitting of bright red blood
- Nosebleed from blood heat
- Coughing of blood with a dry throat
- Bleeding gums with a red tongue

Common adjustments
- Marked Lung heat with cough — add Huang Qin 黄芩 and Sang Bai Pi 桑白皮
- Bleeding heavy — add Bai Mao Gen 白茅根 and Xian He Cao 仙鹤草
- Yin deficiency underlying the heat — add Mai Men Dong 麦门冬 and Xuan Shen 玄参
Contraindications
Not for bleeding from deficiency-cold or from qi failing to hold blood, and not for a patient with a weak, cold digestion — the fresh cold herbs will cause loose stools and worsen the pattern. It is a short-term measure for an acute bleed, not a formula to continue. Any substantial bleeding needs medical assessment.

Pregnancy
Used with caution in pregnancy and only under a practitioner's direction. Sheng Di Huang and the fresh leaves are cold and can upset a pregnant patient's digestion, and although Ai Ye is used in some pregnancy formulas, this combination is not designed for that purpose. Bleeding in pregnancy is an obstetric emergency and must be assessed medically before any herbal treatment is considered.
