Indications
Deficiency-cold of the sea of blood with congealed qi and blood. Cold slows and thickens the blood in the Chong vessel, so the cycle is delayed, clotted and painful.

- Late periods with clots and cramping pain
- Amenorrhoea from cold congealing the blood
- Cramping pain around the navel before the period
- Postpartum abdominal pain from cold and stasis

Common adjustments
- Dryness with a red tongue and warm palms — use the 《金匮要略》 Wen Jing Tang instead — this version is too warm and too moving
- Marked cramping — add Yan Hu Suo 延胡索 and Xiao Hui Xiang 小茴香
- Marked blood deficiency — increase Dang Gui 当归 and reduce E Zhu 莪术
- Qi deficiency with exhaustion — increase Ren Shen 人参
Contraindications
Not for heat patterns with early, heavy, bright red bleeding, and not for yin deficiency with dryness — the formula of the same name is the one for those. E Zhu makes it unsuitable for anyone with heavy uterine bleeding, a bleeding tendency, or on anticoagulant medication. Amenorrhoea should be investigated before it is treated as cold stasis.

Pregnancy
Contraindicated in pregnancy. E Zhu is a blood-breaking herb classically forbidden to pregnant women, and Rou Gui, Chuan Xiong, and Mu Dan Pi all move blood or warm strongly; the combination can cause bleeding and miscarriage. E Zhu also has antiplatelet activity, so the formula should not be taken alongside warfarin, direct oral anticoagulants or antiplatelet drugs, and should be stopped before surgery. Not for use while breastfeeding without supervision.
