Indications
Damp-heat dysentery: damp-heat has lodged in the intestines and blocked the qi there, which is why straining and tenesmus persist even when the heat is being cleared. Clearing and qi-moving must go together.
Clinical Manifestations
- Dysentery with red and white mucus in the stool
- Abdominal pain with tenesmus
- Acute damp-heat diarrhoea with burning at the anus
- Chronic damp-heat colitis with urgency

Common adjustments
- More blood than mucus in the stool — add Di Yu 地榆 and Bai Tou Weng 白头翁
- Food stagnation with foul stools — add Shan Zha 山楂 and Zhi Shi 枳实
- Prolonged with Spleen deficiency — combine with a Spleen-strengthening formula such as Shen Ling Bai Zhu San 参苓白术散
Contraindications
Not for cold-damp diarrhoea with clear watery stools and a pale tongue, and not for chronic diarrhoea of Spleen and Kidney yang deficiency. Persistent bloody diarrhoea needs medical assessment.

Pregnancy
Practitioner supervision required in pregnancy. It is a small two-herb formula and less aggressive than most in this chapter, but Huang Lian is strongly cold and Mu Xiang moves qi, so it is not for unsupervised use.
