Yang-type abscess in the early stage, with toxin and blood stasis: red, hot, hard and sharply painful, before pus has formed. It is a dispersing formula and its window is early — once pus is established, drainage is what the situation requires.

Common adjustments

  • Pus has formed — dispersing is no longer the strategy — the classical response moves to expelling pus, and the modern one is surgical drainage
  • Yin-type flat, cold swelling — not this formula; a warming approach such as Yang He Tang 阳和汤 fits that pattern
  • Any prolonged course — stop — arsenic accumulates, and this pill is a short-term measure only

Contraindications

Xiong Huang is the governing concern: it is arsenic disulfide, and arsenic compounds are cumulative poisons. Acute overdose causes vomiting, diarrhoea, abdominal pain, liver and kidney injury and cardiac arrhythmia; repeated use causes chronic arsenic poisoning with skin changes, peripheral neuropathy and raised cancer risk. Heating converts realgar to arsenic trioxide, which is far more toxic — it must never be decocted, roasted or taken with alcohol that has been warmed over heat, and grinding it dry is itself hazardous. Many jurisdictions restrict or prohibit arsenic-containing medicines outright, and imported products have been recalled over arsenic content. She Xiang is the second concern: absolutely contraindicated in pregnancy, and sourced from CITES-listed musk deer, so genuine musk cannot lawfully be traded in most countries. Ru Xiang and Mo Yao irritate the stomach and move blood, which matters alongside anticoagulants and around surgery. The formula is contraindicated in pregnancy and breastfeeding, in children, in liver or kidney disease, and in any prolonged course; it is not for yin-type cold swellings or for a depleted patient. Finally, an abscess that is enlarging, fluctuant, or accompanied by fever, spreading redness or lymphangitis needs medical assessment — incision and drainage, and antibiotics where indicated — and a facial or neck abscess especially so.

Pregnancy

Absolutely contraindicated in pregnancy and while breastfeeding. Xiong Huang is an arsenic mineral that crosses to the fetus and is documented as harmful, and She Xiang is treated in the classical literature as capable of causing miscarriage.
