Indications
Wind-heat banked up with excess in both exterior and interior— wind and heat block the surface while dry heat accumulates in the Stomach and Intestines, so dispersing, clearing and purging must all be given together.
Clinical Manifestations
- Strong fever with chills, dizziness and a heavy head
- Red sore eyes, bitter dry mouth, sore obstructed throat
- Chest and diaphragm fullness with cough, wheezing or thick sticky sputum
- Constipation with scanty dark urine
- Sores, boils and toxic swellings; urticaria; red nose; bleeding hemorrhoids

Common adjustments
- No constipation — remove Mang Xiao 芒硝 and reduce Da Huang 大黄
- Exterior signs absent — remove Ma Huang 麻黄 and lighten Jing Jie 荆芥
- Skin sores and toxic swellings predominant — add Jin Yin Hua 金银花 and Pu Gong Ying 蒲公英
- Sore red throat predominant — add Niu Bang Zi 牛蒡子 and Xuan Shen 玄参
Contraindications
This formula sweats, clears and purges at the same time, so it is only for a genuinely excess pattern. Not for the elderly or debilitated, not for anyone with a weak Spleen and loose stools, and not while sweating freely. It is a short-course prescription, stopped once the bowel opens and the fever falls.

Pregnancy
Contraindicated in pregnancy — Da Huang and Mang Xiao purge downward, Ma Huang sweats, and the formula also moves blood. Classical texts already warn that it endangers a pregnancy.
