Indications

Moves qi and transforms stasisClears heat and resolves toxinStops the fixed abdominal pain

Qi stagnation and blood stasis in the intestines with mild heat. The knot is there and the pain is fixed, but the burning heat toxin of a later stage has not developed, so the formula unblocks rather than purges.

Clinical Manifestations

  • Stagnation-stage intestinal abscess with fixed right lower abdominal pain
  • Localised tenderness with little systemic fever
  • Early appendiceal inflammation under surgical observation
  • Residual abdominal pain after an abscess has begun to settle

Modern Applications

Common adjustments

  • Heat rising, fever and thirst appearing — the pattern is moving toward Lan Wei Qing Hua Tang 阑尾清化汤
  • Constipation with fullness — increase Da Huang 大黄 and add it late in the cooking

Contraindications

Not a substitute for surgical assessment. Perforation, gangrene, an appendiceal mass with spreading peritonitis, or a deteriorating patient all require surgery, not herbs. Unsuitable for the frail and for patients with a deficiency-cold pattern.

Pregnancy

Contraindicated in pregnancy. Da Huang purges, Tao Ren and Mu Dan Pi move blood, and Chuan Lian Zi and Yan Hu Suo are strong qi- and blood-movers — the whole combination is one classical practice keeps away from pregnancy. Right lower abdominal pain in a pregnant patient is an urgent assessment, not a prescription.
