Indications
Roundworm agitation with heat. Heat in the Stomach and Intestines disturbs the classical picture of restless roundworms, producing paroxysmal pain, vomiting and irritability without the cold signs of the warming variant.
Clinical Manifestations
- Paroxysmal abdominal pain with heat signs
- Vomiting, sometimes of a roundworm, with a bitter taste
- Restlessness and thirst during the attack
- Classical roundworm patterns with a yellow coating
- Abdominal pain with dark, scanty urine

Common adjustments
- Pain very severe — add Bai Shao 白芍 with Gan Cao 甘草
- Constipation with the accumulation — add a small amount of Da Huang 大黄 — a practitioner's decision
- Cold signs rather than heat — the wrong branch — use Li Zhong An Hui Tang 理中安蛔汤 or Wu Mei Wan 乌梅丸
- Marked deficiency in a child or an elderly patient — the parasiticide herbs are reduced or omitted and the deficiency is addressed first
Contraindications
Contraindicated in pregnancy and in deficiency-cold patterns. Not for children, the elderly or the frail without close supervision, since both parasiticide herbs are dose-limited. Lei Wan must never be decocted — heat destroys the enzyme it works through — and must not be used in liver disease without medical advice. Bing Lang is avoided in qi deficiency with sinking, and habitual areca-nut chewing is a recognized carcinogen exposure, which is a reason to keep this herb to a short prescribed course. Severe or worsening pain, jaundice, fever or a worm in vomit or stool means urgent medical assessment: biliary ascariasis and intestinal obstruction are surgical emergencies.

Pregnancy
Contraindicated in pregnancy. Bing Lang drives qi strongly downward and is traditionally prohibited, Lei Wan is a parasiticide whose safety in pregnancy is not established, and is acrid and hot. Parasitic infection during pregnancy is managed by a physician, because the choice and timing of antiparasitic drugs in pregnancy is a medical decision. Also avoided while breastfeeding.
