Indications
Kidney-yang insufficiency: the lower burner's fire fades — cold pain below the waist, water untransformed above or below, the classic strategy: a little fire to generate qi, never a blaze to consume it.
Clinical Manifestations
- Cold sore lower back and knees
- Cold lower body, frequent copious night urination
- Edema or difficult urination of yang deficiency
- Deficiency wheezing (Kidney failing to grasp qi)

Common adjustments
- Pronounced edema — add Niu Xi, Che Qian Zi 车前子 — Ji Sheng Shen Qi Wan 济生肾气丸
- Marked cold — replace Gui Zhi with Rou Gui 肉桂, increase warming
Contraindications
Kidney-yin deficiency with heat signs (dry mouth, red tongue, tidal heat) is the opposite pattern — this warms it wrongly. Contains Fu Zi: professional dosing and preparation are mandatory.
