Indications
Damp-heat sudden turmoil disorder: damp-heat blocks the middle burner so the qi dynamic reverses in both directions at once — upward as vomiting, downward as diarrhoea.
Clinical Manifestations
- Sudden vomiting with watery diarrhoea in hot damp weather
- Epigastric fullness with a bitter sticky mouth
- Restlessness and irritability with fever
- Damp-heat gastroenteritis
- Scanty dark urine with nausea

Common adjustments
- Vomiting is severe — add Zhu Ru 竹茹 and a small amount of Sheng Jiang 生姜
- Diarrhoea is severe — add Ge Gen 葛根 and Huang Qin 黄芩
- Damp outweighs heat — add Huo Xiang 藿香 and Bai Dou Kou 白豆蔻
- Abdominal cramping — add Bai Shao 白芍 and Mu Xiang 木香
Contraindications
Not for vomiting and diarrhoea from cold-damp with a white greasy coating and cold limbs, where warming is needed instead. Acute profuse vomiting and diarrhoea causes dehydration quickly and can be a notifiable infection: rehydration and medical assessment come first, especially in children, the elderly and anyone who has travelled recently.

Pregnancy
Use with caution in pregnancy — it is cold and contains Ban Xia. Vomiting and diarrhoea in pregnancy needs medical assessment for dehydration before herbs are considered.
