Indications
Excess fire of the Liver and Gallbladder at its extreme: not a smouldering constraint but a full blaze disturbing the spirit, with the bowel obstructed. The strategy is to drain hard and give the fire an exit below.
Clinical Manifestations
- Liver and Gallbladder fire with dizziness and tinnitus
- Agitation, insomnia, delirious or manic behaviour
- Constipation with dark scanty urine in a fire pattern
- Red painful eyes, bitter taste, splitting headache

Common adjustments
- Once the fire has broken — stop and change to a gentler formula — this one is not for maintenance
- Blood and yin already damaged — not appropriate; use a Liver-nourishing strategy instead
Contraindications
Only for genuine excess fire in a robust person, briefly. Contraindicated in deficiency patterns, in the frail and elderly, and with loose stools. The classical formula contains She Xiang 麝香, a CITES-restricted animal product that modern preparations omit or substitute.

Pregnancy
Contraindicated in pregnancy — it combines strong purgatives (Da Huang, Lu Hui) with the classical inclusion of She Xiang, which is specifically avoided in pregnancy.
