Ginseng and Perilla Decoction

Indications
Wind-cold in the exterior with qi deficiency and phlegm-damp inside: the surface cannot be forced open because there is not enough qi behind it, and the phlegm underneath keeps the cough alive.
Clinical Manifestations
- Cold in a tired, easily fatigued patient
- Cough with copious white phlegm and no stamina
- Chills and fever with chest fullness and no appetite
- Lingering cold that will not clear in an elderly patient

Common adjustments
- Qi deficiency mild, cost a concern — substitute Dang Shen 党参 for Ren Shen 人参
- Chills prominent, no heat sign — increase Zi Su Ye 紫苏叶 and add Sheng Jiang 生姜
- Phlegm heavy with a greasy coating — increase Zhi Ban Xia 制半夏 and Chen Pi 陈皮
- Heat signs with a sore throat — remove Mu Xiang 木香 and add Lian Qiao 连翘
Contraindications
Not for wind-heat with a red sore throat and thick yellow phlegm, and not for a dry cough of yin deficiency — the aromatic drying herbs would worsen it. Ren Shen is inappropriate while an excess pathogen is strong and unaccompanied by deficiency, since it would hold the pathogen in.

Pregnancy
Use with caution in pregnancy, under supervision. It contains no Ma Huang and no blood-movers, but Ban Xia is traditionally used cautiously in pregnancy and Mu Xiang and Zhi Ke are strongly qi-moving; a practitioner would normally lighten these before use.
