Indications
Excess-pattern accumulation of food and damp-heat with marked qi stagnation. Substance and qi are blocked together in the Stomach and Intestines, and the pattern is unambiguously excess — full, tender, forceful.
Clinical Manifestations
- Substantial food accumulation with a distended, tender abdomen
- Constipation with fullness that pressure worsens
- Dysenteric disorders with severe tenesmus
- Belching, acid reflux and heavy postprandial distension
- Dark scanty urine with a thick yellow greasy coating

Common adjustments
- Dysentery with blood and pus — add Bai Shao 白芍 and Bai Tou Weng 白头翁
- Heat mild, stagnation predominant — reduce Huang Lian 黄连 and Huang Bai 黄柏
- Constitution not robust — omit Qian Niu Zi 牵牛子 and use Zhi Shi Dao Zhi Wan 枳实导滞丸 instead
- Marked abdominal pain — increase Mu Xiang 木香 and add Yan Hu Suo 延胡索
Contraindications
Contraindicated in pregnancy, in deficiency patterns, in the frail elderly and in anyone already weakened by prolonged diarrhoea — it attacks, and there is nothing in it that protects the middle burner. Qian Niu Zi is classed as toxic and is a drastic purgative; E Zhu breaks blood and is avoided where there is bleeding; Bing Lang must stay within its dose limit. Not to be repeated as a routine digestive remedy, and never combined with other purgatives without a practitioner's supervision.

Pregnancy
Absolutely contraindicated in pregnancy. Qian Niu Zi is a toxic drastic purgative prohibited in pregnancy, E Zhu breaks blood and is a classical abortifacient risk, and Da Huang and Bing Lang are both strongly downward-moving. It is also avoided while breastfeeding.
